Case Management Team

OasisOpiate Case Management

Opiate Case Managers provide psychosocial support to people who use opioids through a mobile case management approach. They can help with a number of common issues and concerns such as:

  • facilitating access to regional addiction treatment services
  • assisting clients in applying for income support and housing,
  • facilitating referrals to primary health care and counseling services
  • providing transportation supports for a variety of appointments
  • providing education about the risks associated with substance use
  • training clients in our opioid overdose prevention program (which includes a take-home naloxone kit).

Intensive Case Management Service

Intensive case management services utilizes a Housing First philosophy for people with problematic substance use disorder who are high utilizers of the substance  treatment, criminal justice, and health systems and who have experienced chronic homelessness.
